Library to Help the 75+ Population Register for COVID-19 Vaccination

The Ainsworth Public Library will be a partner with the North Central District Health Department in getting people 75 years of age and over to sign up for the COVID-19 vaccination with the online form. The Library staff will help you fill out the information so that you may get the vaccination.

Many people in this age group do not have a computer or know how to use the internet. Many do not have family or friends that can help them do the online registration. This is why the Health Department reached out to the Library to see if we could help this age group complete the form. The form has basic question such as your name, phone number, city of residence, county of residence, and date of birth. Once you have completed the form online, you will receive a phone call to schedule an appointment. You are asked to please be patient on getting scheduled. Vaccinating 75+ individuals will take the department several weeks.

The Library will also hand out to you the Moderna Vaccination Consent Form which you will complete at home and bring with you at the time of your actual vaccination. The Moderna vaccine is a TWO dose vaccine that needs to be administered 28 days apart. The Health Department will schedule a second clinic after scheduling the first vaccine clinic.

The Library is open Monday, Tuesday, Thursday, and Friday from 11 a.m. to 5 p.m. and on Wednesday and Saturday from 1 to 5 p.m. Please note that right now ONLY those who are 75 and older currently can register for the vaccine. A registry for others will be available in February. Please call the library if you have questions and we will try to help you.
