Questions About the Affordable Care Act? Need Help Completing Your Application?
The Central Nebraska Community Services Navigator for the Healthcare Marketplace will be available to answer any questions regarding the Affordable Care Act or help you complete an application and enroll in health coverage Friday, Dec. 6 12:00pm-6:00pm! Open Enrollment to sign up for coverage through the Marketplace is from November 15, 2014 through February 15, 2015.
If you are wanting to enroll please bring:
- Social Security Numbers or prooof of legal residence for anyone needing insurance
- Proof of tribal membership if American Indian or Alaska Native
- Birth dates
- Proof of income: Pay stubs, W-2 forms, or “Wage and Tax Statements” for everyone in the household
